Outreach
Outreach is a service provided to seniors and their adult children Monday through Thursday, 7:30 am to 3:30 pm. Many seniors living in the community want nothing more than to live out their years in their own homes. In order to do so, some people may need additional supports or equipment, technology, or services that will enable them to do so safely. Following an assessment or interview, Outreach services can connect you with the supports and services that may help you live independently as well as safely in your own home.
These supports may include, but are not limited to;
- Adult Day Health
- Aging Service Access Point
- Caregiver support groups
- Elder Law Attorneys
- Equipment Vendors
- Geriatric Care Managers
- Home Healthcare
- Homemaking services
- Mediation
- Medication Management
- Personal Emergency systems
- SHINE counseling
- Transportation
- Veterans Agent
Outreach can also give you information and refer you to programs that are designed to help make things a little easier in these difficult times, such as;
- Fuel Assistance
- Prescription Advantage
- Supplemental Assistance Nutrition Program and the local Food Pantry
Many people find that their lives would be simplified by relocating to a Senior Community or an Assisted Living facility. Others may require skilled care and will need to move to a Nursing Home. There are many things to consider and it can be helpful to have someone review the points of consideration or even have someone gather the information for you. Outreach can do this at a time when you may have other things on your mind.
Our Outreach Coordinator, Cindy Beard is available Monday through Thursday to assist with the following; support to adult children in the community responding to questions about their parents, assisting seniors in successfully aging in place which can include phone service or home visits, assessing unmet needs in the areas of safety, financial assistance, housing, fuel assistance, nutrition support, and advocacy and other referrals.
